Girl, 13, died after sip of Costa Coffee hot chocolate due to 'failure to follow allergies processes'

An inquest into the death of a 13-year-old girl who died after taking a sip of a Costa Coffee hot chocolate has concluded it was after a "failure to follow the processes in place to discuss allergies".
