Nearly 4,000 patients of a Colorado Springs vaccination clinic told to get re-vaccinated

The Colorado Department of Public Health and Environment shut down Dr. Moma Health and Wellness Clinic's ability to give vaccines late last week.

After an investigation involving the CDC, the state identified problems with vaccine storage and handling, preparation and poor record keeping practices.
